- Be sure your email contains the following attachments: ONE clear, tasteful, digital photo of yourself and an updated resume
- Sign and then fax the accompanying 2009 Ebony Sexy Singles Authorization and Release form to the Sexy Singles Editor at 312-322-1082. Please note that this authorization and release form is the only part of the application that will be accepted via fax.
All forms can be found on www.ebonyjet.com and please, do not send supplementary items outside of what is specifically requested.
We prefer email, but if you must use snail mail, then send all information to the address at the end of this application. Be sure to staple your documents together. Don’t forget to spell check.
