- Include the following attachments: ONE clear, tasteful, digital photo of yourself and an updated resume. This photo should be recent.

- DO NOT send more than one image. Images cannot be emailed separate from application.

- Sign and fax the accompanying 2010 Ebony Sexy Singles Authorization and Release form to the Sexy Singles Editor at 312-786-9606. Please note that this authorization and release form is the only part of the application that will be accepted via fax.

All forms can be found on www.ebonyjet.com and please, do not send supplementary items outside of what is specifically requested.
